Approach
How we can help
Overall, services provided by the team at Lexington Health can help any participant to live life to the best of their ability. We achieve these goals by doing the following:
1. Individualised Assessments
Conducting comprehensive assessments to understand each participant’s specific impairments, functional limitations, and goals.
2. Goal-Oriented Interventions
We provide interventions that are designed to address the participant’s goals based on our assessment. Whether it’s improving mobility, strength, balance, or pain management. These interventions may include exercises, manual therapy, assistive devices, and adaptive techniques.
3. Mobility and Independence
We work on enhancing mobility and promoting independence in daily activities.
4. Pain Management
Many individuals with disabilities experience pain because of their condition or due to secondary complications. We frequently use various modalities such as manual therapy, therapeutic exercises, and pain-relieving techniques to manage pain effectively.
5. Prevention and Education
We educate participants and their caregivers on strategies to manage pain and prevent secondary complications, such as pressure sores, contractures, and muscle stiffness. Providing guidance on proper body mechanics, positioning, and exercise techniques to optimise function and prevent injury for both the caregiver and participant.
6. Assistive Technology and Equipment Prescription
We are able to assess the need for assistive devices and mobility aids, such as wheelchairs, walking aids, orthoses, and adaptive equipment. Providing recommendations for appropriate equipment and ensuring proper fitting and usage.
7. Interdisciplinary Collaboration
Working collaboratively with other NDIS service providers, including occupational therapists, and support workers, to ensure holistic care and optimal outcomes for participants.
8. Regular Monitoring and Review
Treatment plans are regularly reviewed and adjusted based on the participant’s progress, changing needs, and goals. Ongoing monitoring helps to track outcomes and make necessary modifications to optimise outcomes.
9. Home Visits
We provide at-home physiotherapy for NDIS patients who are unable to travel to the clinic. Bringing along with us any small aids that will assist in your treatment. Your health and convenience are our top priorities and we’re happy to assist you in the comfort of your own home.
